Intermediate Care Facilities for Persons with Intellectual Disabilities (ICF/ID) Program
The Daybreak group operates 8 six resident and 1 ten resident ICF-ID group homes in Dallas County. This program provides residential and habilitation services to people with developmental disabilities and/or a related condition.
The ICF-ID program is funded through Medicaid. Financial eligibility is determined by the Texas Health and Human Services Commission. For more information, please visit their website.
Other eligibility requirements:
- Have a determination of an Intellectual Disability (DID) or documentation from a physician of a related condition.
- Meet specified level of care criteria.
- Be in need of and able to benefit from the active treatment provided in a 24-hour, supervised ICF-ID setting.
